Conflict Excusal request Form

Please fill out the following at least TWO WEEKS IN ADVANCE of any minor conflict (Rehearsals) and TWO MONTHS before any major conflict (Major Rehearsal/Dress Rehearsal, Performance).

Requesting an excuse is not a guarantee- please check the syllabus for your performing group on expectations for the year and reasons to request an excusal. Please note that submitting an excusal request after the deadlines of two weeks or two months is an automatic denial, unless there is an extreme extenuating circumstance.

If your request is denied, you’ll be sent an email and told why. If denied, you must make whatever arrangements you can to make the event, rehearsal, or performance. A denial means your presence is imperative.
